Step 3: Waiver and Camp Policy

Han Basketball Academy Waiver Acknowledged

This Release and Waiver of Liability ("Release") is executed on __________ (date) by __________________________ ("Releasor") and Han Youth Basketball Academy, a for-profit organization operating in the State of Maryland ("Han Youth Basketball Academy "). The Releasor as referenced herein includes, but is not limited to, participants, campers, volunteers, coaches, independent contractors, and host families. The Releasor understands that they are responsible for their own insurance coverage in the event of personal injury or illness resulting from participation in or services provided to Han Youth Basketball Academy. Waiver and Release I, the Releasor, hereby release and forever discharge and hold harmless Han Youth Basketball Academy, its successors, and assigns from any and all liability, claims, and demands of any kind or nature, whether in law or equity, arising from my participation with or services provided to Han Youth Basketball Academy. This includes, but is not limited to, claims of bodily injury, personal injury, illness, death, or property damage resulting from my participation or services. COVID-19 I understand and acknowledge that Han Youth Basketball Academy strives to adhere to guidelines provided by leading health organizations and government agencies to protect participants, staff, and the public. However, I recognize that the risk of contracting COVID-19 exists despite these efforts. Should I contract COVID-19 while participating in an HBA activity, I hereby waive any and all claims as outlined in the above waiver and release. Insurance I understand that Han Youth Basketball Academy does not assume responsibility for or provide medical, health, or disability insurance or other financial assistance. By signing below, I acknowledge that I am responsible for my own insurance coverage, and if I am signing on behalf of a minor, I confirm that my child has valid health insurance. Medical Treatment I release and discharge Han Youth Basketball Academy from any claim arising from first-aid treatment or medical services provided in connection with an emergency during participation in HBA activities. If I am signing on behalf of a minor, I verify that my child is in good health and has my permission to participate in HBA programs. I authorize HBA staff to secure medical treatment for my child in the event of injury or illness. I understand that injuries can occur in sports and waive any claims for liability or compensation related to such occurrences. Assumption of Risk I understand that participation in Han Youth Basketball Academy activities involves risks that may result in injury or harm. I voluntarily assume all risks and release HBA from liability as outlined in this waiver. Photographic Release I grant Han Youth Basketball Academy permission to use photographs, videos, or audio recordings of me or my child taken during participation in HBA programs for promotional purposes, provided that my child's name is not disclosed or affiliated with the material. Other Provisions I agree that this Release is intended to be as broad and inclusive as permitted under Maryland law. Should any provision of this Release be deemed invalid, the remaining provisions shall remain enforceable. This Release is governed by the laws of the State of Maryland.

BY SIGNING BELOW, I EXPRESS MY UNDERSTANDING AND INTENT TO ENTER INTO THIS RELEASE AND WAIVER OF LIABILITY FOR KOA SPORTS WILLINGLY AND VOLUNTARILY, AND IF APPLICABLE, ON BEHALF OF MY CHILD. I HAVE READ, UNDERSTAND, AND AGREE TO THE KOA CODE OF CONDUCT POSTED AT WWW.KOASPORTS.ORG AS WELL AS THE COVID-19 GUIDELINES IDENTIFIED HEREIN.

PLEASE READ our Basketball Practice Policy Policy Acknowledged

At Han Basketball Academy, we strive to provide flexibility and transparency while maintaining consistency in our programs. Please carefully review our policies regarding lesson fees and make-up lessons below: Refund Policy All lesson fees are non-refundable. If you notify us of your request to cancel at least 30-days in advance of your registered session, you will receive a FULL refund minus a $25 processing fee. If you notify us of your request to cancel less than 30-days before your session you can transfer your credit to the future lessons or camp. Make-Up Policy To ensure flexibility while maintaining a structured schedule, please review our make-up lesson policy and options below: Make-up lessons must be scheduled within the same season. If you cannot attend a practice, please notify us at least 24 hours in advance to qualify for a make-up lesson. Administration Fee: Any changes of lessons or rescheduling of the original practice plan 2 weeks before session starts will incur a $25 administration fee. Options for Make-Up Lessons We offer a variety of group practices across different age groups weekly, as well as opportunities during Day Camps and Spring, Summer, or Fall Camps. Make-up hours can be used in any available session before each season. To schedule a make-up lesson, please contact us by email or phone.
