HAN BASKETBALL ACADEMY PROGRAM AGREEMENT WITH LIFETIME
This Agreement ("Agreement") is entered into between Han Basketball Academy ("HBA") and the participant ("Member"), regarding the participation in the HBA Program with Lifetime ("Lifetime"). By enrolling in the program, the Member agrees to the following terms and conditions:
1. Membership Fees & Payment Schedule
1.1 All membership fees will be collected on the 15th of each month in advance for the upcoming month.
1.2 Payments will be processed through HBA’s designated payment system.
1.3 Failure to make timely payments may result in suspension or termination of the membership.
2. Membership Termination & Refund Policy
2.1 If the Member wishes to terminate their membership with Lifetime, they must provide a 30-day advance written notice to HBA.
2.2 If the membership fee has already been collected, HBA will pro-rate and refund the remaining amount based on the termination date.
*Example: If a cancellation request is submitted on April 1st, the membership will officially terminate on May 1st, and any applicable refund will be processed accordingly.
2.3 Refunds will be issued within 10 business days after membership termination.
3. Program Administration
3.1 HBA is responsible for collecting all membership fees and handling payments to Lifetime.
3.2 Members agree to abide by all rules and regulations set forth by both HBA and Lifetime.
3.3 Membership fees cover the program costs between HBA and Lifetime, including administrative fees.
4. Liability & Waiver
4.1 The Member acknowledges that participation in the program involves physical activity and assumes all risks associated with it.
4.2 HBA and Lifetime are not responsible for any injuries, accidents, or personal property loss occurring during participation in the program.
4.3 Members must comply with Lifetime’s facility rules and guidelines at all times.
5. Agreement Acceptance
By enrolling in the Han Basketball Academy Program with Lifetime, the Member agrees to the above terms and conditions.
